Silver Lake Pediatrics participates with the vast majority of insurers in our area. The parent or legal guardian accompanying a child is responsible for any copays, deductibles, coinsurance, or fees not covered by insurance at the time of the appointment. It is the responsibility of the parent or legal guardian to verify that Silver Lake Pediatrics accepts your insurance before making an appointment.  Please contact your insurance company directly to determine if Silver Lake Pediatrics is in-network with your family’s plan.

Please remember that co-pays are always due at time of service and that you could be responsible for the complete office visit if your deductible has not been met. For your convenience we accept cash, Visa, MasterCard or Discover.

A last note for new parents. It is vital that you have your new baby added to your policy during the stated grace period. With most insurance companies this gives you thirty days from the date of birth to have the baby added or they will deny your claims back to the the babies date of birth.

Additional Fees: 

• Missed appointments
  • There is a:
    • $25 fee for missed sick appointments
    • $50.00 for Well Child Exams or Consultations that we have not received a 24 hour notice to cancel or failure to show for any of these scheduled appointments. 
• There is an additional $20 dollar fee for completion of forms for use outside of the patient's medical record, including:
  • Day Care
  • Sports Physicals
  • Camp Forms
