To help you become familiar with our practice and prepare for your child’s first visit, we’ve included helpful information on this page. Taking a few simple steps in advance can help your appointment run smoothly and efficiently.
Please provide a copy of the insurance card & drivers license when emailing or faxing the patient packet. If you choose to deliver to our office, we will be glad to make a copy for you.

Our Mission
Our mission is to provide exceptional pediatric care in a conscientious, compassionate manner, in accordance with current standards of care in the United States. Dr. Cheas believes in avoiding antibiotics unless absolutely necessary and embraces a thoughtful, holistic approach to medical care whenever appropriate.
Patient Forms
For your convenience, our office forms are available to download and complete at home. Please print and complete the appropriate forms, then email them to [email protected], fax them to 352-314-2279, or bring the completed packet to our office.
Once we receive the completed packet(s) for each child in your family, we will request your child’s medical records from their previous physician(s). These records will be reviewed and entered into our electronic health record system. After this process is complete, our staff will contact you to schedule your child’s initial visit.

New Patient Packet
Consultations:
Please complete one of the packets listed below that is appropriate for your child.
Return the completed packet packet to our office, in person, mail or email: ([email protected]). Once we receive the completed packet, we will set up a convenient appointment time.
If your child is being evaluated for ADD/ADHD, please ask your insurance company for a list of medications that are covered under your insurance plan. Please bring this list with you.
We require only the child and the parent(s) be present for this appointment.
- ADD/ADHD Consult Forms 11 and Younger
- ADD/ADHD Consult Forms 12 Years and Older
- ADD Follow Up Forms (completed by parent and teacher) required every three (3) months at the time of visit.

What to expect
A pleasant, comfortable first visit builds trust and helps put the child at ease during future visits. We want your child to enjoy getting to know our doctors and staff, so we work hard to establish this bond during every appointment to our office. During your initial appointment, we will explain everything in detail and answer any questions you may have.
